Ideal Candidate:
The Case Manager has a deep passion for non-profit work and feel aligned with the faith-based mission of Several Sources. This position is responsible for the day-to-day management and operation of the organization.
Duties and Responsibilities:
Case Management
- Meet with newly admitted residents to discuss long/short term goals, policies, procedures, etc.
- Provides direct case management services and support to mothers in shelter, such as managing referrals and resolving issues.
- Assist residents in completing government assistance applications (food, medical insurance, childcare, housing subsidies). Applications must be completed within the first 2 weeks after acceptance into the program.
- Collaborates with key partners, including financial, housing, substance abuse treatment, employment, childcare providers, and criminal justice agencies, in accordance with Several Sources Shelters and OMCC policies, procedures, guidelines and applicable state regulations.
- Seek out appropriate enrichment programs for residents such as parenting classes, life skills, church activities, bible study, job certifications, GED, and other educational opportunities.
- Participates in a team approach to treatment including coordinating client services with other Several Sources Shelters staff and other provider agencies.
- Assists in securing housing, gainful employment, education and other referrals in support of client treatment plan.
- Encourages client’s participation in case management services;
- Conduct assessments for children’s’ needs as well and provides appropriate referrals.
- Develops, implements, reviews, revises treatment plans under supervision of Director of Housing.
- Monitors client outcomes.
Documentation
- Initiates timely documentation of services delivered
- Prepares and maintains client charts, records, and reports
- Reviews charts to ensure all necessary documentation is complete
- Documents according to all licensure standards and established policies and procedures
Community Relations
- Develops relations with other providers and referral agencies
- Understands, researches and analyzes community needs
- Uses analysis to provide program direction and goal recommendations
- Advocates on client behalf, policy, and program level
RELATIONSHIPS:
Internal: Maintain verbal contact with Director of Housing and support staff.
SKILLS/KNOWLEDGE REQUIRED:
- A minimum of three to five years’ work experience in non-profit agency operations, specifically in Case Management or Social Work.
- Knowledge of the mission, objectives, policies, programs and procedures and of the principles and practices of non-profit organizations preferred.
- Ability to organize and prioritize projects and time management.
- Must be organized and have the ability to multitask.
- Strong oral and communication skills.
- Proficiency in Microsoft Office, including; Word, Excel, Outlook and PowerPoint.
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT:
High energy level, comfortable performing multi-faceted projects in conjunction with day-to-day activities. Ability to lift at least 5 pounds.
Job Type: Full-time
Pay: $40,000.00 - $50,000.00 per year
