REPORT TO: Program Supervisor
JOB RESPONSIBILITIES:
- Case Manager will meet with all resident’s weekly.
- Facilitate and schedule Individual Service Plan meeting date within 7-days of resident admission into the program with the. expectation for the ISP to be completed before the 30th day of service.
- Monitors the goals and objectives in the Individual Service Plan and Single case Plan.
- Tracks performance indicators/outcomes for each resident.
- Complete referrals for residents to community-based needed resources (i.e., therapy)
- Complete finalize discharge letter to be emailed and mailed to CUA/DHS and/or family.
- Submit schedule and typed progress notes weekly to the Program Supervisor for review.
- Meet weekly for supervision with Program Supervisor.
- Attend all required trainings and meetings. Pursues other training for professional development.
- Collaborate with treatment team members via phone, email and/or face-to-face weekly to gather information on the status of the youth’s progress (i.e., school, medical).
- Record all phone calls, emails, and face-to-face contact with providers in the collateral book.
- Complete HCSIS for AWOL and other significant incidents that are resident and FMKCS related.
- Assist with getting residents enrolled in school via the School District of Philadelphia Re-engagement Center and other educational programs (i.e., AIC, YES Philly.)
- In conjunction with the Program Supervisor, S/he plans for effective programming for young people.
- Ensures that all City, State and County and funding requirements and standards are effectively and efficiently met.
- Transport to appointments as needed.
- Must be able to work one evening per week.
- May perform other duties assigned by the Executive Director.

QUALIFICATIONS:
Bachelor’s degree with two years of case management experience working with youth in the following fields: mental health, juvenile justice, family court or behavioral health.
