The main focus of the Case Manager in Riverside, CA is to support at-risk youth and foster youth in having the resources and services to complete middle and high school. The Case Manager will work closely with the program team and community partners to ensure that each young person has the support they need to succeed. They may also provide tutoring on specific subjects in individual/group settings, and guide students to develop academic and professional study habits. What you will be doing:
- Conduct intake assessments and develop individualized case plans for each young person based on their unique needs and goals
- Provide ongoing support to youth, including advocacy, crisis intervention, and referrals to community resources as needed
- Maintain accurate case records, ensuring that they are up-to-date and compliant with program and funder requirements
- Conduct outreach to school administration, probation department, youth organizations and department of public children family services regarding our program
- Participate in team meetings and case conferences to share information and provide updates on each youth's progress
- Provide guidance and support to youth as they navigate the educational system, including helping with school enrollment, tutoring, and advocacy with teachers and administrators
- Provide job readiness support, including job search assistance, resume building, and interview preparation
- Participate in program evaluation activities, including data collection, analysis, and reporting
- Adhere to all program policies, procedures, and quality assurance standards
- Assisting students with obtaining vital life records (birth certificate, social security cards, etc.)
- Serve as a consistent mentor for an assigned student in group mentoring or one-on-one planned activities.
Qualifications
- Bachelor's degree in social work, counseling, or a related field, OR equivalent work experience preferred
- 2 years of experience working with youth, preferably in a nonprofit or social service setting (preferred)
- Experience working with youth who have experienced homelessness, mental health issues, or involvement in the juvenile justice system is preferred
- Excellent communication and interpersonal skills
- Strong organizational and time management skills, with the ability to manage multiple priorities in a fast-paced environment
- Ability to work independently and as part of a team
- Strong problem-solving skills and ability to remain calm and professional in high-stress situations
- Familiarity with Microsoft Office and case management software

*Must be willing to travel 2 days a week to the Los Angeles office. Gas will be compensated*
Job Type: Part-time
Pay: $23.00 - $28.00 per hour
Expected hours: 25 – 30 per week
