Salary Range: $38.00-43.25 Based on experience level

The GSCH Primary Care Nurse Scope of Practice highlights the unique practice within this nursing specialty.
Nurses practice patient-centered care. Their actions provide protection, health promotion, and optimization of health, prevention of illness and injury; facilitation of healing; and alleviation of suffering through a compassionate presence.
Use nurse knowledge, skills, clinical judgment and critical thinking to determine appropriate actions for a wide variety of patient conditions across the lifespan.
Make autonomous nursing care decisions at the point of care, determine the best nursing care plan for the patient, manage acute and chronic conditions, and evaluate patient response.
Provide nursing interventions that support optimum patient outcomes such as patient education, care coordination, case management and population health. .
Advocates and mobilizes resources within our interdisciplinary team and within the community, to provide safe, high-quality patient care.
Provide input and participate in decision-making, quality initiatives, risk management initiatives that supports our continuous efforts to provide the best care to our patients and reduce health disparities.
Delivering competent care is a team effort. Nurses work alongside medical assistants. They delegate, oversee, provide support and education and role model professionalism.
Communication and problem-solving between team members is essential to delivering quality patient care.
Responsibilities of the Nurse Lead
GSCH is one organization with multiple sites. The Nurse Leads will work together with the MA Nurse Lead daily to assure each location is staffed to deliver safe, quality care.
Collaboratively work towards consistent work flows and promote growth of relationships between sites.
Participate in the day to day clinical care of patients as a primary care nurse as needed.
Competently respond to Code Triage, Code Phone Triage, Walk In Patient Assessment. The Nurse Lead will take the lead role. If not present, the MA Lead or Clinical Director will take on this role. Debrief the event and communicate with Clinical Director and Risk Manager. Assure Incident report is completed if you are the lead nurse, after every encounter of this type.
Schedule all nurses according to agency needs.
Provide solution focused problem solving with staffing across sites. Manage sick calls and time off requests. Address attendance concerns if necessary.
Liaison between nurses and Clinical Director
Review efficiency and quality of care on a daily basis and address concerns in a timely manner.
Nursing team development through recruitment, interviewing potential candidates, and selection of talented, professional nurses.
Support nurse role development, onboarding, cross training, and performance appraisal.
Assure that nurses are appropriately and professionally communicating with patients across the life span. This would include telephone and portal etiquette.
Assure nurses are competent performing all skills such as medication and vaccine administration, nursing assessment, properly addressing med refills, independently manage nurse visits, identify when Provider input is necessary, triage phone calls, respond to Code Triage.
Monitor nurse EMR competency and maintenance of the medical record. Prep patient’s charts prior to appointment.
Engage staff in identifying areas for improvement; invite participation, support agency initiatives and manage change.
Enhance team culture, promote a healthy work environment
Apply evidence-based practice that supports critical decision making
Assure collaborative relationships with and open communication with Providers
Coach, mentor, and assess competency; On-boarding of new staff members
Manage change, involve staff in the process of evaluating work flows, policies and procedures so they have opportunity to be involved
QUALIFICATIONS:
Current New Hampshire Registered Nurse license
Experience in primary care and ambulatory health care setting
Current BLS certification.
