Hospital Liaison Registered Nurse (RN) - PRN
Location: Frankfort, KY
Schedule: PRN (as needed)
As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

About the role:
Educate hospital professional and non-professional staff regarding Bluegrass Care Navigator's (BCN) services and programs. Facilitates communication between BCN and physicians in an inpatient setting. Manages the development, implementation, monitoring, and planning and supervision of strategies to increase referrals. Assists with referrals to BCN. Provides oversight/case management of hospitalized hospice patients. Reports to Executive Director.
Requirements:
- Is a graduate of an accredited school of professional (registered) nursing.

Is currently licensed as RN through the State Board of Nursing.

At least two year’s general nursing experience is required with current hospice, home health, and medical, surgical or critical care experience. Has experience with providing good pain management.

Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.

Complies with accepted professional standards and principles.

Understands BCN philosophy, principles of death/dying.

Understands Medicare Conditions of Participation of a hospice program and all applicable rules and regulations.

Possesses and maintains current CPR certification.

Demonstrated skill in administrative performance that includes expertise with communication, both written and oral.

Public speaking skills.

Must have the ability to work independent of direct on-site supervision.

Demonstrated ability to work well with health care professionals and an interdisciplinary team.

Must arrange travel and be able to access hospitals, nursing homes, and other places of business.

Must be a licensed driver with automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
