Job Overview:
We are seeking a dedicated Physical Therapist to join our nonprofit partner in a new and exciting opportunity in their PACE program. The ideal candidate will provide specialized, preventative medical care to elderly patients, focusing on promoting healthy aging and managing age-related health conditions within the home.

Duties:
- Conducts an initial Physical Therapy assessment
- Develop personalized care plans tailored to each patient's needs and goals
- Establishes and instructs participants and caregivers in safety recommendations, adaptations, exercises, and therapeutic home exercise programs as indicated.
- Provides oversight and supervision of the Physical Therapist Assistants (PTAs) and Rehabilitation Aides in accordance with the Illinois Physical Therapy Act.
- Collaborate with interdisciplinary teams to ensure holistic care for geriatric patients
- Other duties as assigned
Qualifications:
- Degree in Physical Therapy
- 2+ years of working experience, working in senior care
- Must possess valid driver’s license and documentation of current automobile insurance.
- Strong communication skills and empathy when interacting with elderly patients and their families
- Bilingual (English/Spanish) highly preferred
Job Type: Full-time
Pay: $95,000.00 - $120,000.00 per year
