Full salary range: $55,244.80 - $82,867.20
GENERAL PURPOSE OF JOB:
Provides professional meeting facilitation services to families in open child welfare cases and assessments. Provide a variety of routine and complex professional social work duties including areas such as child abuse and neglect, and youth in conflict.
ESSENTIAL JOB FUNCTIONS:

Facilitator Casework:
- Facilitates Family Engagement Meetings using a formal, structured meeting process to promote a consistent, high-quality discussion.
- A trained facilitator offers an impartial, outside perspective.
- Collaborate and coordinate with participants of Family Engagement Meetings as to date, time, and location.
- Collaborates with Assessment or Permanency Caseworker prior to Family Engagement Meeting to discuss content and parameters of the meeting process.
- Consult with families, children, and youth to provide preparation for meetings.
- Collaborates with family and professionals involved in the case.
- Provides detailed, accurate and timely documentation in statewide client information system (TRAILS).
- Provide data internally and to community partners.
- Develop and facilitate 12-week, 3-4 hours per week internship curriculum with Garfield RE-2 and Garfield 16.
- Collaborate with school district enrollment of high school students as participants in the semester prior to the program during student enrollment the prior semester and through program start date.
- Provide and elicit feedback from school districts.
- Follows applicable State Rule and Regulation, County and Department policies and procedures.
- Transports clients as necessary or required.
- Coordinates with supervisors on critical decisions.
- Performs standard casework upon request, including but not limited to:
- Perform on-going needs assessments with families and children/youth.
- Determines what services and interventions will assure the safety in the home.
- Develop and implement a written plan for services.
- Monitor progress according to plan objectives.
- Ensures referrals to services are made.
- Attend and participate in court hearings as needed.
- Completes required assessments and documentation in Trails.
- Performs on call duties and responds to after-hours crisis calls as required including the on-call scheduling rotation.
