Position summary: The LPN is responsible for monitoring the health status of program Participants under the direction and supervision of the Director of Nursing. The LPN acts as a resource for all program staff and home care providers, to act as the Agency’s liaison with the supported individuals’ health care professionals, and to be a health care service advocate for all individuals. Participates as a member of the interdisciplinary team and, as necessary, provides specialized guidance to program staff and/or home care providers around medical/medication issues.
Schedule: Monday-Friday 8am-4pm (40 hr/week)
Essential Functions:

Coordinate medical information for admission and provide a comprehensive nursing assessment for the intake process when required by individual medical condition(s).

Assist in the evaluation of potential home care providers.

Provide participant specific medical training for home care providers.

Complete semi-annual health status reports and individual care plans.

Review and supervise personal care services at least every sixty days, and as needed.

Maintain open communication with other service providers.

Monitor placements to ensure compliance with all applicable regulations.
Additional Responsibilities:
-
Be a medical resource to program staff and home care providers.

Monitor the accuracy of health-related records including medication, seizure activity, significant incidents, etc.

Participate in staff, team, and in-service meetings to ensure consideration of, and provide information about, medical and health issues.

Provide nursing care training as assigned.

Provide on-call consultation and site visits as required.

Monitor medical care to ensure that consumers receive quality care, treatment, and respect in the community.

Responsible for the medical portion of programs annual quality assurance and evaluation review.

Monitor compliance with any medical related operational changes that result from annual review.

Maintain Individual Confidential Files with all required documentation.

Timely and accurate coding for services into electronic medical records system for billing purposes.
Physical Requirements:
-
Ability to sit/stand for extended periods of time.
-
Ability to exert light to moderate physical effort which may involve lifting up to 25 pounds, pushing and/or pulling objects of similar weight.
Qualifications:
-
Current licensure to practice as a Licensed Practical Nurse by the Commonwealth of Massachusetts Board of Registration of Nursing

A valid Driver’s license with acceptable driving record, acceptable CORI and current RN licensure are required throughout the duration of employment with The Arc of Opportunity

Proven ability to utilize electronic health record systems to monitor compliance (currently iCentrix and HCSIS)

A minimum of two years prior experience with elderly or disabled persons
