Annual Wellness Visit Nurse Practitioner
REPORTS TO: Chief Medical Officer and Supervising Physician
EDUCATION & EXPERIENCE: BA/BS, graduate of AMA accredited Family Nurse Practitioner program. Current or pending FNP license, current CPR certification.
SKILLS: Provide professional health care services for clients in an ambulatory clinic setting.
PRIMARY DUTIES:
- conduct Annual Wellness Visit (AWV) visits for Medicare patients following established guidelines.
- develop and implement personalized care plans based on AWV findings.
- collaborate with the healthcare team to integrate AWVs into the overall patient care strategy.
- must have a genuine desire to continuously learn and provide high quality patient care.
- seeing ambulatory patients of the Center as scheduled and assigned by Center.
- maintain full privileges for practice of clinical medicine at Family Health Center.
- abide by the ethics of the medical profession and all federal, state and local laws and ordinances applicable to his/her practice.
- participate in committees/work groups as assigned.
- may participate in quality improvement projects.
- other duties as assigned.
OTHER DUTIES:
- assist in the Center’s provision of health education and health promotion programs including but not limited to such groups as the Center’s patients, physicians, staff, and/or community residents.
- other duties as assigned by the Medical Director and/or Center management.

- occasional exposure to electrical, chemical, gaseous and dusty conditions
- occasional exposure to patients with communicable diseases
- understand/carry out simple/detailed, oral/written instructions
- memorize and retain instructions
- read and interpret detailed specifications
- able to respond quickly and calmly in medical emergencies
