Position Title: RN Case Manager
This is not a remote position. This role requires you to live within our service area which includes Allegan, Barry, Berrien, Branch, Calhoun, Cass, Eaton, Kalamazoo, St. Joseph and Van Buren counties.

As an RN Case Manager, you will provide skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Centrica Care Navigators.
Here’s what you will be doing:

Continually assess and evaluate patient status.

Initiate and apply appropriate preventative and therapeutic nursing procedures and techniques.

Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given.

Assist the patient with the activities of daily living.

Work with the patient/family/caregiver and other team members in providing continuity of care.
Work schedule:

4 (10-hour days) or 5 (8-hour days), Monday-Friday

Weekend and holiday rotation
Requirements:

Graduate of an accredited school of professional (registered) nursing.

Maintain license as a Registered Nurse from the State of Michigan.

Minimum of one-year general nursing experience with current Hospice, medical, surgical, or critical care experience.

Current BLS certification.

Experience with issues concerning the terminally ill, and death/dying, and pain and symptom management.

Maintain a valid Michigan driver’s license and the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Negative TB skin test and/or chest X-ray and other tests as required by Centrica Care Navigators.

Up to date on all routine immunizations required by LARA and agency policy.
