The Client Care Coordinator (CCC) is a salaried, remote position and is an intricate part of the overall success in addressing any/all Client scheduling needs and Caregiver assignments. The CCC is at the heart of operations and helps in assuring overall best Client experience and outcome. Reports directly to the Executive Director. A CCC will be responsible for, but not limited to, the following:
- Overall, Client intake/start of care experience• Client onboarding, coordination of initial scheduling/start of care and ongoing schedule• Works with Account Executive/Executive Director in assessing Client’s needs• Assigns Caregiver based on Client’s needs• Acts as the Client’s primary contact in addressing any/all scheduling (change in hours/days), staffing caregiver needs/concerns and invoicing questions/concerns • Manage on average 1,000 Client Hours per week• Maintaining professional, effective and consistent communication with Client/Caregivers• Create and maintaining Client’s schedule/calendar • Works with Account Executive in initiating/creating DocuSign documents for Clients, if/when needed • Audits and confirms reception of all necessary Client documents (service-agreements and payor source)• Critically assess Client’s needs to assign best Caregiver • Proficiently and thoroughly complete data entry within EMR/AxisCare• Experience with Outlook/Teams and Shared Drive preferred• Confirm Caregiver schedules/shifts and clock-in/outeveryday• Certifies Caregiver Care-Notes and Completion of ADLs• Maintains and works to limit any overtime and bonus offers extended to Caregivers • Responsible for any before/afterhours support or coverage, as needed • Weekend on-call rotation (1-2/month with coverage bonus)• Attends daily stand-up/down calls• Submitting an EOD report if/when deemed necessary by management
- High school diploma or equivalent • At least one year of job-related experience in administrative, intake/scheduling and caregiving preferred• Certified Caregiver with CPR/First-Aid, Food Handler’s Card & TB Test/Xray• Fingerprint clearance card
