Summary
The Enhanced Case Manager at Sunny Glen Children’s Home is responsible for the Enhanced Case Management services for our Supervised Independent Living (SIL). The Enhanced Case Manager will be responsible for providing guidance and assistance to young adults who are transitioning to independent living in our SIL program. The Enhanced Case Manager utilizes Trust Based Relational Intervention (TBRI) to help client’s problem-solve social, emotional, and mental health issues. Assists with crisis intervention, de-escalation, and resolution. The ECM must maintain accurate documentation on case files, comply with DFPS Minimum Standards Policy and Procedures, and establish state and federal standards.
Essential Duties and Responsibilities
· Assist with case management operations in the Sunny Glen Children’s Home-San Benito location, as needed.
· Maintain a caseload and accurate records per agency policies and procedures, licensing standards, and/or funding requirements.
· Conduct initial intake of child/young adult to assist in the orientation process.
· Monitor individual’s services, ensure all required services are delivered, set initial appointments, and communicate appointments (psychological evaluations, CANS/ANSA, Circle of Support meetings, counseling sessions, dental/medical/vision checkups, school enrollment, and visitations) with appropriate staff and/or CPS/DFPS.
· Meet face-to-face at least monthly with the young adult and have at least weekly contact to monitor their progress to make suggestions and recommendations on the young adult’s goals.
· Provide the young adult’s caseworker with written initial goals to be accomplished in the SIL by the 30th day of SIL placement.
· Provide the young adult’s caseworker updates and progress being made during the review of the CPOS.
· Provide the young adult’s caseworker when ECM services are no longer necessary or should be discontinued due to the lack of progress.
· Provide ECM young adults a minimum of two times a week, face-to-face contact during the first 30 days with a planned reduction in weekly contact as skills develop and self-sufficiency increases.
· Provide ECM young adults daily contact by phone, email, texting or other type of contact.
· Review and report to DFPS of the young adult’s progress and ability to be able to successfully function in the SIL placement without ECM services will be conducted as a part of the Child’s Plan of Service (CPOS).
· Assist the young adult in scheduling, obtaining, and maintaining available medical, educational, employment, or other services through community-based providers, governmental agencies, or other organizations.
· Assist the young adult in scheduling their medical and dental appointments and arranging transportation to necessary appointments.
· Develop and monitor a medication management plan that assists the young adult in understanding, scheduling, and managing their medication; and assist the young adult in improving their daily life skills such as cooking, money management, cleaning, and shopping.
· Ensure proper completion of assessments and deadlines as required per internal policy and procedures.
· Coordinate and complete referrals, service planning, and client documentation of services provided (case notes, admission assessments, incident reports, progress notes, service plans, monthly reports, monthly surveys, etc.).
· Communicate with the CPS caseworker regarding visitations, Individual Service Plans (ISPs), court dates for the individual, any medical, dental, and school updates on the individual, and any updates in the individual’s case.
· Prepare, compile, and submit accurate and timely case files in case management software and continually assess ongoing changes in behavior, circumstances, or conditions that may affect a child’s/young adult’s safety.
· Complete appropriate discharge planning before the individual’s discharge.
· Participate in case staffing with assigned supervisor residential assistant, and/or other staff.
· Act as a liaison with stakeholders, including CPS, CASA, and other entities.
· Work some evenings, after hours, weekends, and holidays as needed to assist with campus emergencies, crises, child placements/young adult intakes, or ongoing reunification process as requested by the supervisor.
· Maintain confidentiality in all areas.
· Attending training and meetings as scheduled.
· Must abide by Sunny Glen Children’s Home's mission of giving nurturing, trauma-informed care to children/young adults from difficult backgrounds.
· Learn and abide by Sunny Glen Children’s Home policies and procedures (and minimum standards).
· Learns and abides by TBRI and Handles with Care -Ability to evacuate/travel if needed due to natural disaster.
· Assist with case management operations in the Sunny Glen Children’s Home-San Benito location, as needed.

Qualifications

Education and/or Experience
A master’s degree in social work or other human services from an accredited college or university is required.
Experience providing support services to youth and young adults who may have experienced child abuse and neglect, are in foster care, are homeless, are at risk, or have mental or behavioral health challenges.
Experience providing independent living services to youth and young adults.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, and the general public.
Certificates, Licenses, Registrations
Valid Texas Driver's License with an excellent driving record.
Have a record of tuberculosis screening showing the employee is free of contagious TB.
CPR/First Aid, 15-passenger van, Handle with Care, TBRI, case management software, Paycom, and other DFPS-mandated training (training offered on the job).
