Job Summary:
We are seeking a highly skilled and compassionate Pharmacist to join our healthcare team. The ideal candidate will have a strong background in pharmacy practice, including knowledge and the ability to manage medication therapy in a virtual setting. This is a dynamic role that requires a deep understanding core elements of Medication Therapy Management. The successful candidate will be responsible for providing exceptional virtual patient care, ensuring the safety and efficacy of medications, and collaborating with other healthcare professionals to achieve optimal outcomes. This role is not a hourly paid position, it is a per member per month model. Pharmacists will get paid based on the number of patients that are assigned to their panel.
Responsibilities:
- Counsel patients regarding optimal medication use and health conditions.
- Create strategies along with the patient to promote safe use of medicines.
- Provide un-biased information about medications, and treatment.
- Clarify any medication/treatment related questions that patients may have.
- Develop care plans along with the patient to manage conditions and make recommendations to their Healthcare providers as needed.
- Create a patient-specific Medication Action Plan (MAP) and written/ verbal communication and feedback to healthcare providers.
- Conduct medication reconciliations to identify any medications, to add or delete or change.
- Make recommendations on eight types of drug-related problems* as identified under the Medication Therapy Management process *Eight types of drug-related problems targeted in MTM: untreated indications (indications without therapy); improper drug selection; sub-therapeutic dosage; overdose; adverse drug reactions; drug interactions; failure to receive drugs, excessive therapy (therapy without indication).
- Conduct follow-up calls, for targeted interventions and adherence checks as necessary.
- Monitor patient alerts on the we-based pharmacist platform, such as a delay in taking medication or a change in lab values.
- Take appropriate action based on shared clinical decision-making.
- Assist the member/client in communicating medication related needs directly to the primary care provider/specialists.
- Collaborate with case managers and caregivers as needed under the continual care model for the patient.
- Generate reports on interventions as needed.
- Communicate professionally and discuss/document clinical recommendations/interventions to prescribers and interdisciplinary care teams.
- Ensure patients and family members have a positive experience by providing a prominent level of customer service by always responding promptly, professionally, and politely.
- Meet productivity and performance expectations set forth by the manager.

1. Proficient in use of Microsoft Word, Excel, Outlook, Teams. Ability to learn and use proprietary web-based pharmacist platform.
2. Able to multi-task using multiple computer databases/platforms/applications.

Qualifications:
- PharmD (MANDATORY)
- Washington DC licensure (MANDATORY)
- Completion of an accredited PGY1 and/or PGY2 pharmacy residency program preferred.
- MTM training certificate from a recognized/accredited program OR 3 years of experience in delivering MTM services directly to patients strongly preferred.
- Spanish, Amharic languages a plus
- Other certificate programs a plus (e.g., CDE)
